Transcend Insights Launches Unified Solution Suite for Population Wellness
Unified solution suite includes focus on wellness, patient engagement and future app marketplace to redefine how population health is managed, measured and implemented.
CAMPBELL, Calif. – February 23, 2016 – Transcend Insights®, a population health company, today announced the launch of a unified solution suite, built on its technology platform HealthLogix™. The new offering, which expands upon Transcend Insights’ population health platform for community-wide interoperability and real-time analytics, will provide health system executives, care teams and patients with a full suite of solutions designed to help them transition from episodic to proactive care for long-term wellness.
“Our vision for population wellness is to disrupt the ‘sick-care cycle’ that represents the majority of health care delivered today,” said Marc Willard, President of Transcend Insights. “In most contexts, population health refers to managing care for the sick patient – wellness is not a part of the equation. We see this as a vastly untapped opportunity to make an impact on people’s lives and their health. By addressing health and wellness with our HealthLogix platform, and by connecting disparate systems with open standards, we will make it easier for care teams to do their jobs and deliver a better care experience.”
Transcend Insights’ unified solution suite includes:
- HealthLogix Populations™ – A performance measurement application that provides actionable, configurable visualizations and dashboards to help health system executives and care teams quickly evaluate quality and utilization metrics, and compare performance across an entire care community. Users can access comprehensive data visualizations, such as facility performance comparison views to identify opportunities to improve health outcomes.
- HealthLogix Care – A tablet and web-based application that provides physicians and care teams with a single, comprehensive view of a patient’s clinical, claims and wellness data at the point of care. Complementing electronic health records, this solution generates timely, proactive insights that support confident decision making for a more engaging patient care experience.
- myHealthLogix – Powered by Patient IO, a leading care coordination platform, myHealthLogix is a patient engagement solution that connects care teams with their patients by providing collaborative care plans, secure messaging, medication management capabilities, and tailored educational information to enable a more coordinated approach to care.
- HealthLogix Market – A future offering that will establish an open, scalable marketplace for third-party health care applications developed on the HealthLogix platform. HealthLogix Market will enable access to valuable data and insights via Fast Healthcare Interoperability Resources (FHIR) Application Programming Interfaces (APIs) to add another level of technical customization and increased benefit to existing HealthLogix platform clients and partners. Patient IO is among the first HealthLogix Market partners. In addition, Transcend Insights has partnered with Chiron Health, an organization that provides secure video visit technology and Medisas, a workflow and communication platform for hospitals.
“We are excited to be offering our care coordination platform, Patient IO, through Transcend Insights’ myHealthLogix application,” said Jason Bornhorst, Chief Executive Officer and co-founder of Patient IO. “We believe Patient IO will bring a unique patient experience to Transcend Insights’ population wellness platform and will ultimately drive better outcomes.”
Hardin Memorial Health, an integrated delivery network, has selected the HealthLogix platform and its Populations and Care applications to connect and transform their organization’s data into actionable insights that their system executives, care teams and patients can use to improve outcomes and promote wellness.
“Hardin Memorial Health chose to integrate Transcend Insights’ HealthLogix platform, Populations and Care applications to achieve our population health goals and optimize our community engagement capabilities,” said Tom Carrico, Vice President of Operations at Hardin Memorial Health. “We are excited about tracking our quality measures and the overall health of our patient population within a single, comprehensive view. Having the tools and connectivity to better know and engage our community will help us improve the patient experience and reduce the need for costly hospitalizations and emergency room visits.”
