The Journey to Simplify Population Health Begins with
Transcend Insights™ at HIMSS15
Combining interoperability and health care analytics to streamline and simplify population health, new mobile capabilities increase physician and patient engagement in health decisions
CAMPBELL, Calif. – April 9, 2015 – Transcend Insights, a population health management company, will demonstrate its HealthLogix™ platform and unveil new mobile point of care solutions at the HIMSS15 Annual Conference and Exhibition in Chicago, Illinois (booth #2048). Transcend Insights helps make the journey to population health easier by simplifying the complexities of data collection and analysis so physicians and care teams can spend more time engaging with their patients. The HealthLogix platform provides a single, comprehensive patient view to easily identify individuals in need of health improvement opportunities. Population health management is essential to improving the quality of care in value-based reimbursement models. Today, physicians and care teams face various challenges that impede efficient and effective care, such as a lack of community-wide interoperability and analytics. Transcend Insights’ HealthLogix platform connects disparate electronic health record systems and transforms both clinical and claims data into real-time Actionable Health Intelligence® to enable better health outcomes and more informed decisions at the point of care. Insights are made available via a web and mobile application. Mobile accessibility allows clinicians to securely and conveniently review and manage community-wide patient data while focusing their time where it matters most, during their patient interactions. Transcend Insights will demonstrate the following new capabilities at HIMSS15:
- Plan of Care – Communicates a patient’s care plan to his or her primary care physician or care team, and tracks patient health and wellness goals along with dates to measure progress.
- Gaps in Care – Organizes gaps in a patient’s care, such as missing procedures, labs or medications, and prompts the clinician to intervene.
- Drug-to-Drug Interactions – Displays adverse drug interactions pertaining to a patient’s active medication regimen, and classifies the interaction in order of severity.
- Patient Referral – Streamlines the patient referral process while enabling physicians to continue functioning within their existing workflows.
“Physician-patient engagement is a critical component of successful population health management,” said Marc Willard, President of Transcend Insights. “By making comprehensive and visually compelling clinical insights available on a mobile application, we are helping to restore the overall health care experience. As a result, patients can become more informed and active participants in their care to achieve their best health.” For more information about the HealthLogix platform, please visit Transcend Insights at HIMSS15 in booth #2048.
